HRS acquires Rimidi to expand virtual care capabilities in chronic disease management

Deal adds ambulatory care, diabetes management, continuous glucose monitoring integration, and workflow interoperability to HRS’s remote care platform

Health Recovery Solutions (HRS), a remote patient monitoring and virtual care platform company, has acquired Rimidi, a chronic disease management and remote monitoring company focused on diabetes and cardiometabolic care.

The acquisition adds Rimidi’s ambulatory care expertise, EHR and workflow integration capabilities, and continuous glucose monitoring (CGM) functionality to HRS’s broader remote care platform. The companies said the combination is intended to support healthcare organizations seeking to manage multiple chronic conditions through a single virtual care partner.

HRS said the deal builds on its recent expansion beyond remote patient monitoring into longitudinal and chronic care management. The company launched its PatientFirst Pathways model in 2025, aimed at supporting connected care across different stages of the patient journey, and in 2026 expanded further into chronic care management.

Rimidi brings experience in chronic disease management programs in ambulatory settings, with a particular focus on diabetes and cardiometabolic conditions. According to HRS, the acquisition is designed to help providers manage care across a wider range of conditions while integrating data and workflows more directly into clinical operations.

The companies said the transaction addresses three areas of demand among healthcare organizations: scaling remote patient monitoring across multiple conditions, supporting longitudinal care models, and reducing the number of vendors involved in care delivery technology.

Under the combined platform, HRS said providers will be able to incorporate Rimidi’s diabetes management tools, including CGM integrations and clinical workflow support, alongside HRS’s remote care infrastructure and post-acute care capabilities.

“HRS is committed to expanding enterprise remote patient care to meet evolving customer needs,” said Jason Comer, CEO of HRS, in a statement. He said the acquisition strengthens the company’s ability to support longitudinal care and adds expertise in diabetes management and clinical workflows.

Dr. Lucienne Ide, founder and CEO of Rimidi, will become chief medical officer of HRS. In a statement, she said the combined organizations will support patient care across care settings, conditions, and acuity levels.

HRS said Rimidi was selected in part for its interoperability capabilities and its experience in chronic disease management. The company cited Rimidi’s work integrating CGM data from devices including Dexcom, Libre, and Eversense into EHR-based workflows.

The companies also pointed to clinical results from Rimidi’s platform, including an example in which one healthcare partner reported an average 2.8% reduction in hemoglobin A1c levels and lower hospitalization rates among high-risk patients with type 2 diabetes.

HRS said current customers will gain access to expanded diabetes and clinical management capabilities through the combined platform, while Rimidi customers will gain access to HRS’s enterprise remote care infrastructure, post-acute care programs, and operational support services.

The companies said integration will continue throughout 2026, with support teams in place to limit disruption to existing patient care programs.
